Digital mental health interventions in emergency contexts

Introduction: Humanitarian crises have increased the global prevalence of psychological disorders, overwhelming care systems and highlighting existing gaps in access to care. Objective: To compile evidence from 2018 to 2025 regarding the design, efficacy, adaptability, and governance of clinical mental health interventions in crisis contexts. Methods: A structured narrative review was conducted using searches in PubMed, SciELO, PsycINFO, Web of Science, and Google Scholar. Peer-reviewed studies, technical reports from international bodies, implementation guidelines, and impact studies were analyzed, highlighting interventions characterized by clinical validation, an equity-oriented approach, and community participation. Results: The effectiveness of interventions mediated by digital tools depends on community digital literacy, integration with primary care networks, and professional supervision. Barriers to inclusion extend beyond connectivity to encompass cultural, linguistic, socioeconomic, and digital competence factors. Interventions featuring "offline-first" architecture, participatory co-design, stepped-care models, and ethical data governance demonstrate higher adherence and sustained symptom reduction. Conclusions: Technology enhances human intervention when grounded in digital equity, ethical governance, and a well-organized healthcare ecosystem. Public policies are required to prioritize connectivity infrastructure, digital skills training, sensitive data protection, and continuous impact assessment.
